How it works

Activates both GIP (glucose-dependent insulinotropic polypeptide) and GLP-1 (glucagon-like peptide-1) receptors simultaneously. GIP receptor activation works synergistically with GLP-1 signalling to enhance insulin secretion, reduce glucagon levels, slow gastric emptying, and suppress appetite through central nervous system pathways. The dual mechanism produces greater appetite suppression and metabolic improvement than GLP-1 agonism alone. Clinical trials demonstrate that tirzepatide's GIP component may enhance fat cell lipolysis directly — an effect not seen with GLP-1-only agents — contributing to its superior fat loss outcomes.

Research summary

TGA-approved in Australia as Mounjaro (type 2 diabetes) and Zepbound (chronic weight management). The SURMOUNT-1 trial (2022) is the pivotal efficacy study: 2,539 adults without diabetes achieved an average 20.9% body weight reduction at the highest dose (15 mg) over 72 weeks — significantly exceeding semaglutide's 14.9% in the STEP 1 trial. A head-to-head trial (SURMOUNT-5, 2024) directly compared tirzepatide to semaglutide and confirmed tirzepatide's superior weight loss at equivalent therapeutic doses. Currently the most effective approved pharmacological weight management option available in Australia.

How this connects to your protocol

On GLP-1 protocols, hitting protein targets is your most critical nutritional variable. Semaglutide suppresses appetite — the risk is eating too little protein and losing muscle alongside fat. This number protects your lean mass.

Targets are evidence-based estimates. Consult your practitioner for personalised advice.

Risks & side effects

Nausea, vomiting, and diarrhoea during titration (similar to semaglutide but often milder)
Potential pancreatitis risk — discontinue if abdominal symptoms develop
Weight regain on cessation — similar trajectory to semaglutide
Not PBS-listed for weight loss in Australia as of 2025 — significant out-of-pocket cost
Contraindicated in personal or family history of medullary thyroid carcinoma
